Accurately perform and record clinical observations such as blood pressure, temperature, respiration, and pulse rates- Engage positively with patients/carers/families and staff members promoting mutually respectful and appropriate relationships
- Communicate effectively with patients regarding providing and planning their care and carrying out risk assessment needs as required
- Communicate complex and sensitive information verbally and in writing.
- To act as patient advocate during practice or PCN meetings, sharing any relevant information confidently and accurately, maintaining patient confidentiality appropriately
- Develop an understanding of MDT structures and members roles
- Support patients to improve and maintain their physical, mental, and behavioural health and well being
- Provide advice to patients including promoting self management where appropriate and signposting patients to other PCN colleagues, local community, and voluntary sector services
- Maintain accurate and contemporaneous patient health records
- Skills and training needs to be updated through continuous professional development, attending regular educational updates, via e learning and face to face sessions as appropriate.
- Work in adherence to NMC Code for Nursing Associates, within own level of competence and the protocols and policies of the practices/PCN
- Participate in regular appraisals
- Maintain a good working knowledge of Health and Safety procedures, fire safety and mandatory training as per practice and PCN policies.
- Work flexibly to meet the needs of the service and patients.
