Description

An Exciting opportunity has arisen for a motivated, innovative and engaging Clinical Practitioner to provide clinical expertise and operational support to the new Virtual Ward model within BCHC, this will be to support admission avaoidance and supported discharge from the acute settings.


The post holder will be able to provide expert clinical expertise, operational management, to the Virtual Wards Model within Birmingham Community Healthcare (BCHC) NHS Foundation Trust.


The purpose of the role is to work across the system in a variety of city-wide settings but not exclusive to the community and inpatient units, and to in-reach into secondary care, to identify and assess patients, to prevent avoidable hospital admissions and facilitate early supportive discharge.


Provide all aspects of direct clinical patient care, provide expert clinical support and advice and is able to manage aspects of the service within teams which are fundamental parts of this role.


Assess, diagnose, prescribe, and manage a highly specialist adult patient caseload in a variety of settings, which will include assessment, diagnosis, treatment planning therapy programmes and discharge of patients.

Provide a comprehensive health assessment based on a specialist body of knowledge.


Use an extended scope of practice beyond own profession, review all information available, including interpretation of laboratory and radiological results.

Recognise patients with changing health status and take appropriate actions.

Initiate pharmacological intervention, as an independent prescriber or using patient group directives, administer medications as prescribed by others in accordance with current legislation and the Birmingham Community NHS Foundation Trust (BCHCNHSFT) policies.


Plan and organise complex activities for patients, including liaison with other hospital services and external agencies, and detailed planning for patient management, referral, and discharge.


Work with all services to provide dedicated support to the patients to prevent avoidable hospital admissions and facilitate early hospital discharge.
