THIS POST IS FIXED TERM FOR 24 MONTHS TO MEET THE NEEDS OF THE SERVICE.Are you looking to be part of an exciting team within the health board? Are you driven to improve the care and experience of all age patients who access Planned care services? Are you looking for an opportunity to play a pivotal role in Betsi Cadwaladr University Health Board's, first point of contact for patients services?
As a Single Point of Contact (SPOC) and Self Care Service Co-ordinator, you will provide a high level of service co-ordination to the Single Point of Contact team, which forms the basis of the 3P's delivery.

The ability to speak Welsh is essential for this post.
The post holder will enable and promote the effective use of data, information, knowledge, and technology to improve, inform and support service improvement projects and initiatives as identified.
A key priority will be to ensure the accurate and timely reporting of key data as required by the service, health board, partner agencies and Welsh Government.
The coordination of service policies and protocols ensuring these are up to date and centralised will be a key role.
The successful post holder will be able to demonstrate an effective management style, encouraging and supporting innovative thinking from staff within the service and other departments, service users, carers and families.
The successful post holder will train to deliver Self Management Education as part of the role in addition to having the responsibility for planning, timetabling and undertaking preparatory work for meetings and to provide any necessary documentation.
