An exciting opportunity to join our Home First Acute Discharge Team / Community Hub Team. The role will be based across two sites (Hospital and Community). We are a multi-disciplinary team covering the whole of Salford facilitating clients discharge back into their own home. We are looking for an experienced Band 7 Home First Team Leader to join our service.

You should have significant post registration experience including working in an Acute or Community and setting, covering a wide variety of clinical areas.

You will be a excellent team player and also possess excellent communication skills and must be able to work on your own initiative to organise and prioritise your own caseload and balance this with the demands of this diverse and varied role.

Your workload will include being the 'coordinator' of the day in the Home First Community Hub.


The post holder will work closely with the Multi-Disciplinary Team consisting of Therapists, Social Workers, Pharmacists, Mental Health, Voluntary Sectors, Housing and Admin supporting the development of the discharge services to deliver care closer to home.

Working as part of the multi-disciplinary team, driving the changes to Hospital discharge. The post holder will have the opportunity to work across the discharge services.


Supporting Hospital Discharge the role of the Home First Team Leader is to advise and support the Home First Community Hub Team and the Home First Acute Discharge Team by providing information and specialist knowledge to patients, their families and carers.

To act as a resource to the multi-professional team. Supporting a Home First strength based approach to patients being discharged from the Acute hospitals.

  • Proactive member of the discharge team
  • To work as part of the multidisciplinary team to meet the aims and
objectives of the Trust with regards to discharge planning and bed
management.

  • To be actively involved in multidisciplinary discharge planning
meetings with patients and other professional members of the multi-disciplinary team.

  • To maintain a high clinical profile, to support and advise wardbased


staff on discharge issues, to empower and advise ward-based staff to be more actively involved in discharge planning and access care pathways, where appropriate.


  • Facilitate quality discharge ensuring the Discharge Policy is adhered to.
