Description

We have a fantastic opportunity for enthusiastic and experienced individuals who are confident communicators with strong organisational skills. These are new and exciting roles which will be pivotal to our district nurse/community teams in building and strengthening their relationships with GP Practices within their aligned Primary Care Networks (PCNs)

There are a total of 5 posts available and you will be based in a Community Nursing team in one of the 5 localities across Birmingham:

North locality (Based in Langley Community Nursing Team)
East locality
West locality
Central locality
South localit

Please note that over the next 6-12 months there will be changes to community nursing teams across the East, West, Central and South localities as part of a programme of transformation therefore although you will be based within a locality your base may change when in post.
