Project Team Leader - Health and well being

ME2 Strood - Hybrid and on site and various GP locations

£25, hour week


An exciting opportunity to join a well established, vibrant and inclusive National charity who support home based, unpaid carers to continue living their lives to the fullest.


Did you know that young and adult carers often don't put themselves first and that their mental and physical health can often suffer? Our client is a charity that is driven to support carers to get the best support available to them.


This role is to engage all primary care networks (PCN) in Medway, working directly with GP's helping them to identify and support carers within their practices.


As team lead you will be responsible for implementing a project to identify and raise awareness of carers and to offer health checks to them and promote carers first services as well as signpost to other services available in the community.

As the health and well being team lead you will

  • Build a network across the 10 PCN's in Medway and Swale, covering 100+ practices and 30,000+ patients per PCN
  • Identify unknown carer, offer health checks and signpost to additional services for further support
  • Engage and identify a Carers champion in each PCN and support them by arranging carers awareness training to them and all practice staff
  • Provide day to day line management for 2 team members
  • Ensure KPI' are achieved
  • Use EMISS database to provide evidence for reporting, quality monitoring and overall evaluation of the impact of the project
Skills and experience

  • NVQ level 3 in a health/social qualification
  • People management/leadership experience
  • Experience of working with projects within the NHS or charities or voluntary sector
  • Great communication skills
  • Good IT skills, database and Microsoft Applications

£25,500.00 per year
