A fantastic opportunity has arisen for an efficient, organised Care Coordinator to join a highly reputable, established and rapidly expanding Domiciliary Care Business.

Adada Care Services are looking to recruit a Care Coordinator to support their head office function in Capenhurst, Chester.

We are looking for a dynamic and independent professional with solid care and people management experience and a flexible, enthusiastic, and can-do attitude to join our fast and progressive team

Due to the nature of the role, the applicant must be a driver with their own car.


Job Details

  • Salary: £25,000 Plus on Call Payments
  • Hours of work: Monday
  • Friday
  • On call: 1 week in 4 on a rota basis (on call allowance is paid)
  • Responsible to: Registered Manager
  • Responsible for: Senior and Care Workers
The successful applicant will work closely with the Care Coordinators to plan the service effectively to ensure we offer a robust high-quality service that values both clients and staff through one-to-one supervision with the staff, buddying new starters and holding regular patch meetings


The role of the Care Coordinator is to support the delivery of the highest quality care support services by line managing care workers including field base work carrying out a range of functions in the field.

You will be liaising with our clients directly both at the commencement of the service and for its duration and will be responsible for an assigned location to assist in all aspects of care management.

This a front facing role and you will be responsible for client engagement and ongoing compliance. Some of the main duties include will be expected include;

Job Role

  • To assist the Registered Manager in all aspects of care management, contributing to the development and growth whilst ensuring commitment to quality at all times
  • Working in accordance with the quality governance programme, for example completing spot checks, appraisals, supervisions, competencies, medication and care plan audits as per the governance schedule.
  • To maintain accurate records in line with policies and procedures relating to the handling, storage and if necessary, deletion, of personal details to ensure compliance with the General Data Protection regulations for Care Workers and Service Users.
  • Assess clients needs, write robust care plans and risk assessments that meet the clients needs using an outcomebased approach that is personal centred
  • Complete HR duties such as investigations and complete succinct incident and accident reports
  • Line manager Senior and Care workers. Accountable for the organisation of shadows, introductions, and sign offs etc
  • Work closely with the recruitment team to ensure sufficient current and future staffing levels meet the needs of the business
  • Work closely with stakeholders and other professionals to ensure the needs of the clients are met
  • Liaise as necessary with partner professionals and organisations (e.g. social workers, GPS, nurses) in respect of developing and delivering holistic, personcentred packages of care
  • Building and maintaining excellent working relationships with our Service Users and their families as well as multi agency partners such as District Nurses, GP's, CHC and Social Workers
  • Make necessary referrals for example to falls prevention and OT teams
  • Work closely with referring bodies such as Local Authorities, CCG's, Age Concern etc
  • Deal with queries from clients and families in a calm and professional manner.
  • Organise and coordinate Care Services.
  • Supporting career engagement and welfare
  • Be present across the patches to support and train new and existing staff
  • Investigate incident and accidents within your patch, providing detailed incident forms
  • Carry out oncall coordinating and response duties outside normal office hours as agreed including providing front line care when required
  • Support with the rota scheduling when required
  • Carry out any other duties deemed necessary for the successful operation of the business
