A vacancy has arisen for a Nurse Associate to join our Community Nursing Team at Claughton Community Nursing Team, based in Claughton Medical Centre to work within the Wirral Community Health and Care NHS Foundation Trust.

It is a Full time post hours per week.
Working shifts and over 7 days.

It is a permanent post.

You must have the Nursing Associate Foundation degree and be on the NMC Register.

We are looking for someone who is skilled in assessing and planning nursing care.


The post holder acts professionally at all times and uses their knowledge and experience to make evidence based decisions and solve problems.

They recognise and work within the limits of their competence and are responsible for their actions. They contribute to the provision of care for people, including those with complex needs.

They understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a person and their family, and their responsibilities in relation to communication and collaboration.


The post holder will improve the quality of care by contributing to the continuous monitoring of people's experience of care.

Identify risks to safety or experience and take appropriate action, putting the best interests, needs, and preferences of people first.

They will contribute to on-going assessment and can recognise when it is necessary to refer to others for reassessment. The role requires the ability to be flexible and adaptable whilst working in people's homes and other community settings. The post holder will have some responsibility for supervising less experienced or qualified staff and students in community placements. They will be expected to actively contribute to quality assurance processes and service development.
