Social Work England Registration
Extensive Adults Social Work Experience
Experience of working with older people, people with disabilities, and long term health conditions and their carers.
Knowledge of the Care Act and Mental Capacity Act

This local authority based between London and Kent is seeking an experienced
Social Worker to be part of
Hospital Avoidanceteam based in Queen Elizabeth Hospital in Woolwich.

The team will assess and support the person during the period of crisis, ensuring a plan is in place to continue appropriate supportwhere required, aiming to prevent admission into an acute hospital bed.

The remit of the team is to support residents to remain at home during periods of health or social care crisis where a 2 hour response is often required.

The team operates a 7 day a week service and you will be expected to work a 5-day week including weekendson a rota basis once a month.
