£ £16.96 dependant on experience
Hours:
Mon-Fri 30 Hours Per Week
Length of the contract: 3 months initially
Grafton Recruitment are now seeking to appoint a Children's Continuing Care Case Manager to our public sector client based in Lincoln.
The Children's Continuing Care Case Manager will have excellent clinical knowledge as well as communication and assessmentskills; be resilient and able to work autonomously but also work as part of a team.
They will ensure that health care needs of children eligible for Continuing Care are identified and me.They will work as part of a multidisciplinary team with external agenciesto ensure children's health care needs are supported appropriately.
Duties of the Children's Continuing Care Case Manager
- Supporting the Clinical Lead Nurse and Head of Continuing Healthcare in the development of a range of services for children with complex health needs.
- Undertaking specialist assessments for children with complex needs in relation to care packages, respite care and residential care placements.
- Collating and undertaking comprehensive and coordinated assessments for decision making for Children's Continuing Care as part of a multidisciplinary team with families and health and social care professionals.
- Promoting Personal Health Budgets and assist families to utilise their Personal Health Budget in accordance with national guidelines.
- Providing professional Nursing advice to Acute Hospital Trusts, Residential Homes and Social Services with regards to Continuing Care and Personal Health Budgets.
- Attending a multidisciplinary panel where arranged to present cases and provide clinical information.
- Case managing continuing care packages, ensuring timely reviews are conducted and ensuring care packages continue to be effectively commissioned and are cost effective.
- Assisting in maintaining a database for all patients assessed for continuing care.
- Manage unpredictable and challenging situations, acting in a sensitive manner when communicating to children and families, recognising that communicating the information may include settings that are highly emotive, emotional and challenging.
- To deal with reviews and/or complaints from families/carers, staff and the Social Services authority in a timely manner.
- Undertake research and development as required to support the continued improvement of the service.
- Supporting families to contribute to and be confident in the Continuing Care process, by encouraging their participation in a transparent and equitable process; ensure this is accurately documented as part of the assessment.
- Ensures credible evidence is documented fully to support decision making and later review of the case through audit and/or independent review.
Experience Required
- Active registration as a registered Children's Nurse.
- Holds a UK driving licence and has access to a vehicle insured and taxed.
- Experience caring for children with complex needs.
- A background that demonstrates a clinical experience in a children's healthcare setting including interagency and multiagency.
- Good knowledge of NHS and Social Services guidance and Continuing Cares national framework.
- Able to demonstrate strong knowledge base around safeguarding children and consent.
- Evidence of mentorship.
- Clinical knowledge base in order to ensure scrutiny of compliance with statutory guidance and best practice.
- Experience of managing risks and reporting.
- Experience of managing the performance of a team without supervision.
- Evidence of continued professional development
- Expert knowledge of Continuing Care pathway including screening, assessment and decision making processes, review and current appeals.
- Knowledge of adult and child safeguarding process and responsibilities.
- Aware of Caldicott principle and information governance.
- Understanding of Confidentiality and Data Protection Act.
- Understanding of the public sector.
- Awareness of equality and valuing diversity principles.
- Demonstrated capability to plan over short, medium and longterm timeframes and adjust plans and resource requirements accordingly.
- Experience of setting up and implementing internal processes and procedures.
- Ability to prepare and produce concise yet insightful communications for dissemination to senior stakeholders and a broad range of stakeholders as required.
- Negotiation and conflict management skills and the ability to influence in formal settings.
- Demonstrated capabilities to manage own workload and make informed decisions in the absence of required information, working to tight and often changing timescales.
- Competent IT skills, including the use of Microsoft excel and Microsoft office.
- Advanced theoretical and practical knowledge of a range of work practices and procedures e.g risk management, protection of vulnerable children, DOLs, MCA, information governance etc.
- Plan and organise a number of meetings, panels and case conferences.
- Ensuring mandatory training is up to date.
