About The Role:


We are looking for a Patient Pathway Coordinator to join our welcoming and diverse Administration team based at our Practice Plus Group Hospital, Shepton Mallett.


You will work as a member of the administration team to provide a comprehensive administration role to the Unit, ensuring that all patients are dealt with in a professional and courteous manner, which endorses the patient centred approach of the team.

You will assist in the clerical function of the Unit.

Working proactively to ensure that Outpatient clinics and Theatre lists are filled to capacity at all times and report any shortfalls and of any difficulties experienced with the booking of patients and the capacity available to the Booking Lead & Patient Referral Manager.

This role is on a fixed term contract to cover maternity leave

Shifts for this role are Monday - Friday, 9am - 5.30pm

What you'll be doing:


This will include taking ownership of the patient journey and pathways within a team of pathway co-ordinators - working together on certain specialties.

You will be responsible for booking all types of appointments, answering phone queries and reviewing opportunities within the team to get patient pathways moving quickly.

You will provide an efficient and effective booking service to patients.

Answer telephone calls from patients, GP surgeries and other hospital staff on all aspects relating to booking appointments

Providing advice in all matters relating to patient's appointments, both by telephone and face to face.

Contacting patients on internal waiting lists to offer earlier appointments.

You will process and manage referrals in order to meet the requirements of Practice Plus Group performance objectives.


Helping to book theatre and outpatient sessions within strict booking guidelines adhering to breach dates and ensuring optimal utilisation of clinic lists.

Operating an efficient Patient Administration System (PAS) to ensure the timely and accurate recording of administrative and clinical information.


Also monitoring daily out-patient clinic and theatre lists and ensure all patient notes are prepared appropriately and delivered to the relevant departments and units in advance of the appointments, and to review these lists to ensure they are up-to-date.


What we'll look for in you:

You will have recent and relevant experience, who has excellent communication and interpersonal skills as well as the ability to work under pressure.

Just as important though is your flexibility, belief in teamwork and your ability to use your initiative, follow instructions and prioritise a busy workload
