This is an exciting opportunity to become a part of a growing community based therapy team.


The Calderdale Community Rehabilitation Teams provide a time limited period of assessment, treatment and rehabilitation according to patient centred goals for people aged 18 years and over.

The main base is at Bracken Bed view care home. The team also cover contracted discharge to assess beds across Calderdale.

The post holder will work as part of the multi-disciplinary team comprising Occupational Therapy, Physiotherapy, Rehab Assistants (RA) and Local Authority staff.

Flexibility working across the service from different team bases may be required depending on the needs of the service.


The post holder will undertake highly skilled support work with people who are an temporarily staying in an intermediate care bedded unit.

Patients will have had an illness or injury resulting in a temporary or permanent disability and loss of function in daily activities and who will be frequently in pain and require advice and support with their rehabilitation plan.


Working to agreed protocols and guided by standard operating procedures, the post holder will be key in the treatment and care of patients and also their carer/relatives where relevant.

The post holder will be working under indirect supervision of the registered practitioners in the team.

This will involve taking responsibility for an identified and allocated caseload following screening/assessment by a registered practitioner and will involve a degree of autonomy depending on the complexity and risk of procedures being undertaken.


Activities will include:


  • Assessing to protocols.
  • Implementing treatment to protocols.
  • Monitoring, evaluating and adjusting treatment within the scope of protocols.
  • Assisting registered practitioners with complex treatment.
  • May involve assessing for, ordering and issuing basic equipment/orthotics.
  • May include leading and assisting with group exercise and advice sessions.
  • May take the responsibility for the training of others, including delivering training.
  • May involve delegating tasks to others.
  • May involve supervising the routine tasks of others.

The main responsibilities will be:


  • Completing hollistic patient assessments
  • Completing Multi factorial Falls risk assessments
  • Creating patient centred goals
  • Creating delegated exercise programs
  • Identifying a patient problem list and creating a patient centred treatment plan
  • Completing functional assessments i.e. personal care and kitchen.
  • Access, Home and Discharge patient visits.
  • Working alongside a larger MDT to discharge plan.
