We are looking to recruit a Qualified Band 4 Assistant Practitioner to join our Mental Health Liaison Team (MHLT) based at the Queen Elizabeth Hospital (QEH) in King's Lynn.

West Norfolk MHLT provides 24/7 assessments and interventions in all areas of QEH.

Working alongside medical team, mental health nurses and practitioners, you will work a 24 hour shift pattern including unsociable hours i.e.

some weekends and bank holidays.


The post holder will be responsible for assessments, risk management, care planning, and co-ordination of care delivery within a well-resourced team, supported with regular supervision and providing opportunities to work in partnership with health and social care, linking into the wider multi-disciplinary service.

The post holder will also provide cross departmental working within the acute pathway as required

Please note that Band 4 Assistant Practitioners require:
A Care certificate or completed within first 12 weeks of start date, or relevant equivalent qualification.


AND


Foundation Degree in Health and Social Care / Mental Health at Level 5 or Apprenticeship in Health & Social Care / Mental Health Level 5.
