About us
Melbourne & Chellaston Medical Practice is a very busy, high achieving practice with 15,500 patients, based across 2 sites. We offer an organised and supportive work environment with patient care at the heart of everything we do. Our clinical IT system is SystemOne. There are
5 partners, 3 Salaried GP, 1 ANP, 2 Practice Pharmacists. Our experienced nurse team comprises 3 nurses and 1 HCA.

Job responsibilities

Main duties of the job


As a Trained Nursing Associate, you will work under the supervision of and in partnership with the Senior Nurses to provide and deliver a high standard of nursing care.

You will contribute to the on-going assessment, providing and monitoring care based on evidence and working in partnership with the multi-disciplinary team


Other Duties
You will be part of the nursing team and duties will include.

  • Venepuncture.
  • Height, weight & BMI checks.
  • Lifestyle advice.
  • Blood pressure checks.
  • ECGs.
  • Wound care.
  • Measure & interpret blood glucose levels.
  • Effectively share information and check understanding about long term conditions.

Person Specification

Qualifications

  • Essential_
  • Qualified Nursing Associate with Registration (or working towards completion of the
Nursing Associate Apprenticeship programme)

  • Demonstrate an ability to undertake baseline physical observations and record results correctly. In addition, be able to identify anomalies that should be referred to a first level nurse or one of the medical staff
  • Desirable_
  • To engage in positive relationships with patients/carers/families and other staff based on respect and within appropriate boundaries.
  • To act in a professional capacity at all times.
  • To be able to set therapeutic limits with patients, carers or relatives.
  • To be able to communication with the patient about care planning risk assessment needs.
  • Communicate complex and sensitive information verbally and in writing.
  • Actively contribute during handovers to meet the care needs of patients.
  • Actively contribute to meetings
  • To formulate care plans under the supervision of registered staff.
  • To communicate risk issues as they arise to ensure safe care.
  • To communicate concerns relating to patient behaviour or mental state to other members of the multidisciplinary team

Knowledge and Skills

Essential

  • A Registered Nursing Associate on the NMC Register
  • Educated to a minimum level 5 Foundation degree level specific to nursing associate practice
  • Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
  • Experience of working in teams under appropriate supervision as part of a multidisciplinary team
  • Excellent interpersonal and communication skills IT skills appropriate to the role
  • Demonstrate evidenced based knowledge in the main physical health problems that adult patients may present with i.e., high blood pressure, diabetes, obesity, epilepsy, respiratory issues, constipation, lack of personal hygiene, including foot care.
  • Demonstrate an ability to complete ongoing monitoring templates and health assessments with the support from other members of the team.
  • Analytical Skills
  • To understand the concept of verbal and nonverbal communication and be able use the skills at the desired level.
  • Has an understanding of risk behaviours and risk management.
  • Understanding of incident reporting.
