Description

As an experienced practitioner the post holder will lead on the continuous professional development of the respective workforce, supporting the Service Manager with the development of the services performance capability, undertake or co-ordinate service improvement and governance initiatives, supporting the development of policy and practice and deputise for the Service Manager.

To develop & implement education & training programmes to Youth Justice Service teams on generalist health issues to raise awareness of impact on young people.

To ensure that the transition process to adult health provision for young people meets individual health care needs and is appropriate, timely & well-co-ordinated.

To work in partnership with the multi-agency team of professionals including Childrens Services, Education & CAMHS to help young people known to the Youth Justice Service reach their potential by helping them to understand their health & take responsibility for their health and wellbeing appropriate to their age & development.


The post holder will work with young people and families within the home environment, school or other identified community settings.

A key aspect of the role is to work flexibly to meet the needs of the service as required.
