How you will make a difference

As a Specialist Healthcare Navigator, you will work seamlessly with the other health professionals involved in the service users care, and with other Care Navigators and Apprentice Health and Wellbeing Coaches, to ensure holistic and meaningful support is provided to those with health vulnerabilities on their release from prison, ensuring continuity of care and fruition of release plans.

You will use a coaching framework, to manage wellbeing in the pre and post release period, to prevent a return to ill health as well as a reduction in reoffending.
