This is an exciting opportunity for an experienced Health Practitioner Assistant/Health Care Assistant to join the Integrated Community Reablement Assessment Service (ICRAS)


We have a number of posts working around Liverpool and South Sefton providing care and support for patients who require support with keeping them at home rather than admitting them to hospital or patients who have been in hospital and require support to ensure that their assessments for care are made in their own home rather than hospital.
