The Role
No two days are the same for a Hospital Liaison Worker; the work we do is hugely varied.

Here is a taster of what your role will include:


  • Supporting family, friends and unpaid carers ('carers') when either themselves, or the person they care for, has been discharged from hospital;
  • Carry out initial assessments with carers to help establish needs arising from their caring role, and using a strengths approach to work with them to support these needs;
  • Delivery of intensive 1:1 wellbeing support for carers for up to 6 weeks' after discharge from hospital;
  • Undertaking Carers Assessments for carers who still have eligible needs under the Care Act at the end of Hospital Services support;
  • Conducting ward rounds within the acute hospitals in Devon (virtual and facetoface), on a rota with others in the team.

The Person

Previous experience/qualifications:

  • Evidence of experience of 1:1 support work within the Health & Social Care field/ or relevant role.
  • Evidence of an understanding of carers and their needs under the Care Act.
  • Excellent communication and interpersonal skills, with the ability to empathise with the needs of unpaid carers;
  • Good knowledge/understanding of unpaid carers and their issues;
  • Evidence of experience of assessing and meeting client's needs (including emotional) through support planning over short term interventions.
  • Evidence of commitment to working in a way that is carerled and carerfocussed, ensuring services are of the highest quality.
  • Evidence of excellent administration, organisational and recordkeeping skills, and ability to write in a clear, concise and professional manner.
  • Car driver/owner (or otherwise able to satisfy requirements for travel associated with the role).


A previous understanding of the Care Act 2014 is desirable, but a willingness to learn and understand about the Act is what really matters.


  • DO I NEED EXPERIENCE IN THE ADULT SOCIAL CARE SECTOR?_


Ideally - yes, but if you can bring the majority of the above requirements to the table, we'll give you the tools to learn and develop in the role.

We can deliver a full program of training to ensure you're able to flourish in the role - all you need to do is demonstrate that you have the ability and essential skills we are looking for.
