We are looking to recruit a Carers Information & Support Officer to cover the South and East Staffordshire area. This role is to proactively identify Carers and provide them with information, advice and support including supporting them to access services, peer support and short break opportunities in order to reduce the impact of their caring role

The role is home based; hybrid working between home and the community of South and East Staffordshire.

In this role, you will be expected to:

  • Induct new Carers to the service, meeting them at their home or in the local community
  • Undertake person centred assetbased assessments with Carers to help understand the impact of their caring role, provide information, advice and guidance, and develop support plans that help to reduce this impact
  • Support Carers through periods of transitions, including working closely with Young Carers Practitioners
  • Undertake regular reviews of Carers needs / support plans
  • Record & maintain accurate data confidentially to the online central management systems
  • Signpost and refer Carers to relevant support
  • Support with selfhelp techniques i.e. utilising national and online resources, connecting with family and friends
  • Inform Carers of their rights under relevant legislation to support them to make informed decisions
  • Support Carers to understand the condition of the cared for person to enable them to provide appropriate care including signposting to appropriate service i.e. dementia services, mental health services
  • Undertake community engagement activity to identify hidden Carers
  • Deliver carers awareness training / briefing sessions to community members and professionals
  • Facilitate community drop in's
  • Support the development / facilitate support groups, peer support and short break opportunities for Carers i.e. coffee and chats, activities and social events
  • Promoting and safeguard the welfare of children and adults
