Key Responsibilities:

Provide highquality personal care and dedicated support to clients.
Implement care plans and risk assessments to ensure clients' individual needs are met.
Maintain accurate and uptodate records for seamless care coordination.
Foster effective communication with clients, families, and healthcare professionals.

Requirements:

A minimum of 6 months previous paid experience as a carer or support worker.
Ability to commit to a minimum of 24 hours of work per week.
Access to your own car is essential for this role.
