Job Summary:

  • Undertaking assessments and reviews of individuals' care and support needs
  • Developing and implementing care plans, ensuring they are personcentred and promote independence
  • Coordinating and liaising with relevant professionals and agencies to ensure the best outcomes for service users
  • Providing advice, guidance, and support to individuals and their families
  • Monitoring and reviewing the qualityofcare provision

Qualifications and Requirements:


  • NVQ Level 2 in care or a similar relevant qualification.
  • Experience in assessing and supporting the needs of vulnerable adults.
  • Sound knowledge of relevant legislation and best practices in adult social care.
  • Excellent communication and interpersonal skills, with the ability to build positive relationships with service users, colleagues, and external stakeholders.
  • Strong organizational and time management abilities, with the capacity to manage a varied workload effectively.

£20,297.16-£32,044.36 per year
