Description
Our Organisation is looking for a Care Coordinator to join our client's healthcare team.

As a care coordinator, you will be responsible for managing elderly and disabled patient's care, overseeing treatment plans, collaborating with other health care professionals.


Responsibilities:


  • Organise the care team, training, and schedules
  • Ensure staff are aware for health and wellbeing policies
  • Managing rotas/schedules. Keeping accurate records.
  • Maintaining good working relationships with customers and Care Workers.
  • Understand individual's needs and treatment plans
  • Ensure patients have access to highquality health information
  • Coordinate multidisciplinary teams
  • Maintain patient confidentiality and quality care
  • Provide care to patients if required or to cover shifts.
  • Skillset of a Health Care Professional
Skills and requirements

  • Appropriate trainings
  • Excellent organisational skills
  • Decision making and planning skills
  • Relevant work experience
  • Awareness of patient safeguarding process

Essential:
Own Car with Driving License

Happy to move to Shepton Mallet
