The Bristol Dementia Wellbeing Service is a partnership between Devon Partnership NHS Trust & Alzheimers Society delivering a service across Bristol for people living with dementia and their families.

We provide seamless support from diagnosis to end of life, helping people to live as well as possible, for as long as possible, in their own community.

There is also a focus on increasing public awareness of dementia and improving access for hard-to-reach communities.


The role


An exciting opportunity has arisen within the service for a qualified clinician to develop into the role of a Senior Dementia Practitioner.

In this role you will be part of a team providing support for older people with dementia, their carers and relatives across a defined area of Bristol.


The successful post holder work alongside the GPs and/or Care Homes to offer specialist support and guidance and individual therapeutic interventions.


We are looking for a passionate person who has an interest in developing in this exciting role, making a genuine difference to peoples experience of using dementia support services.


Main duties of the job

Responsibilities include:

  • Working alongside GP Practices and/or Care Home staff in promoting the best outcomes for people with dementia
  • Holding a caseload of people with dementia to offer support at points of transition along their journey, ensuring timely and proactive intervention
  • Working closely with the persons support network to identify signs of change to intervene early to avoid crisis
  • Working collaboratively with other services to advise and ensure proactive support
  • Supporting a diagnosis of dementia and providing long term case management for those with a diagnosis of dementia
You will be part of the multi-disciplinary team, working collaboratively and in receipt of regular clinical and caseload supervision.

You will be supported to identify and access training for the role and induction will include our service developed competency framework for working with dementia.


You will join an exciting, innovative and award-winning service, which embraces change and consistently seeks to promote quality and psychological approaches in all aspects of our service delivery.


The core working hours are Monday-Friday 9am-5pm however the service currently operates Monday to Friday 8am-6pm and so requires flexibility to work 8am-4pm or 10am-6pm as part of the service Access Point.


All clinical staff are provided with laptops to enable ease of flexible community working and for home working purposes and are flexible to work across the city of Bristol as required.

Person Specification

Qualifications

Essential

  • A professional qualification and registration eg RMN, RGN, LDN, Social Work Degree or equivalent, BSc/Diploma in OT, SROTor Community Nursing Degree

Desirable

  • Non Medical Prescriber and/or another recognised therapy qualification
  • Post graduate qualification relevant to specialism
  • Clinical Supervision qualification
  • Training to level 3 in Safeguarding Adults and children and young people: roles and competencies for Health Care staff

Experience

Essential

  • Substantial experience of managing clinical complexity.
  • Working within a community setting in either the statutory or voluntary sector.
  • Experience of lone working and able to make clinical decisions in
