Description

Due to expansion of the service, we are looking to recruit an additional RA Planner to work within Community Intermediate Care Service (CICS).

You will work within an office based team who plan and coordinate care for patients within their own homes.


Your role within that team will be to schedule care calls for the Advanced Clinical Support Workers to both Active Recovery and Stroke patients using an electronic system.


You will be responsible for the entry of patient data ensuring that appropriate care is delivered to patients by the most appropriate member of staff.


We are looking for a highly motivated and organised individual who is logical in their approach to a challenge, and has a keen eye for detail.

You must have attributes that include resilience and be proactive.


You will have experience of working within a health service or administrative role, and be competent in IT related tasks and systems.


It is desirable that you will have experience of scheduling work for a team however, this is not essential as full training will be provided.


The role is highly responsive to changing demands for care and you therefore need to be flexible to meet this demand.

You will be expected to covershifts from 6am to 8pm, 365 days a year including weekends and Bank Holidays.


This vacancy is advertised on a fixed term contract basis for a period of 12 months due to the post being part of an organisational change within Integrated Community Care.

The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts and the management of organisational change.


Active Recovery CICS support patients by providing care and clinical support at home to avoid hospital admission or facilitate discharge from hospital.

It is a citywide service made up of a multi-disciplinary team; consisting of Nurses, Physiotherapists, Occupational Therapists, other specialist therapists, Pharmacists, Advanced Clinical Support Workers, Admin support and Managers.

The service operates from 6am to 12am, 7 days per week, including all Bank Holidays.
