Social Care Assessor - Permanent

An exciting opportunity has arisen for a Social Care Assessor to work in the Community Rehabilitation and Short-Term Assessment Team, based at the Memorial Hospital.

We are looking for a highly motivated individual who wants to develop their skills and is looking for a new challenge in the workplace.

This role requires the postholder to undertake Care Act Assessments, Mental Capacity assessments and be involved with Safeguarding Enquiries. Knowledge and experience of a strengths-based approach to assessment is essential.

The Community Rehabilitation and Short-Term Assessment Team is an integrated service with Oxleas NHS Foundation Trust and has access to Physiotherapy, Occupational Therapy and Therapy Assistant Practitioner resources.

It has a primary focus on prevention and supporting a person to optimise their independence - with a view to reducing or eliminating the need for services in the long term, making the best use of resources available and support people to self-manage their care needs.
