B8a Pharmacist required by NHS Trust based in the Midlands
To start ASAP - Three months +
Full time working hours


Duties:

Screening SACT Chemotherapy for Haematology Day Unit.
Screening Oral Chemotherapy from Haematology Clinics including using the various portals to register PAFs.
Being an authorised Pharmacist in the Aseptic Unit and releasing products.

Covering the Haematology Ward and provide pharmacy ward service including but not limited to; Drug history taking, Medicines Reconciliation, Processing inpatient and discharge medicines, Authorising Chemotherapy protocols and patient counselling.

Passing the assessment to gain entry on the Trust Intrathecal Register.
