Description
Social Work role in an integrated Health and Social Care Team that covers short-term rehabilitation and Reablement for service users recently discharged from hospitals, rehab units and onto community therapy services specialising in stroke and neuro rehab
Intermediate Care Southwark is recruiting a qualified social worker (role not suitable for newly qualified social workers) to support the team with - completing Care Act assessments
- reviews of long-term care provision
- mental capacity assessments relating to care support, change in accommodation and finances
Experience and skills preferred in areas of rehabilitation settings, neuro and stroke and parkinson services, hospital settings and familiarity working in multidisciplinary teams
