The aim of the service is to support our residents and people on the recovery pathway, so they lead more independent lives in the community, safely and to the best of their abilities.

The postholder will work with individuals, assessing their needs, co-producing goal orientated support plans. The postholder will work in conjunction with Community Mental Health Teams participate in multi
- disciplinary team meetings to achieve positive outcomes for working age adults.

We are seeking an enthusiastic and committed colleague to join our friendly team, the successful applicants will be expected to be imaginative and resourceful.

They will have a positive and optimistic view of the process of change.
