We have an exciting opportunity for a Social Prescribing Link Worker to join our Pharmacy team at West Midlands Regional located in the West Midlands
Salary
This position is full time, you'll receive an annual salary up to £ 27000
_ _per annum - depending on experience.

What you'll be doing:
As a Social Prescribing Link Worker, your responsibilities will vary. You will
- Complete initial assessments with patients referred to and eligible for the social prescribing service. Assessing their current wellbeing and identification of what matters to them.
- Work with patients to develop personalised wellbeing plans based on services and activities that offer practical, social and emotional support that will improve their health and wellbeing.
- Manage a caseload of social prescribing patients providing followup support and ensuring they are able to access 'prescribed' services with confidence.
- Be responsible for the scheduling and sequencing of patients social prescribing activities.
