Description
The post holder will be a registered Social Work Practitioner working as a part of the multidisciplinary Specialist Child and Adolescent Mental Health Service (S-CAMHS) within Hywel Dda community based teams, undertaking specialist mental health assessments and therapeutic interventions to a wide range of clinical presentations.
Base to be mutually agreed within the 3 county provision of Carmarthenshire, Pembrokeshire and Ceredigion. The post holder will provide two days of clinical activity to the Ty Helig team in Aberystwyth.If this results in travelling across bases, the post holder will be able to claim expenses in line with health board policy and travel within their agreed working hours.
The post holder will offer and provide a liaison and consultation service to social workers and other key agencies working with children and young people who are Looked After, Fostered or Adopted who have been referred into S-CAMHS.
The post holder will be effective in robust communication and attendance at local authority multiagency meetings.
The post holder will require a degree of professional autonomy and will be required to account for their professional practice through appraisal and audit.
Provision of specialist consultations to social workers and other professionals within key agencies in understanding the mental health needs of Children/ Young people within the context of their lived experience and family context when they may under a Looked After/ Foster/ Adoption status.

- Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire; Tregaron, Aberaeron and Cardigan in Ceredigion; and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire;
- 48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;
- Numerous locations providing mental health and learning disabilities services;
- Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales
The ability to communicate in Welsh is desirable for this post.
