The Integrated Community Services Division are holding a Nursing and AHP open day on the 24thMay 2023 at Crompton Health Centre, 12pm - 6pm.

We are looking for 2 enthusiastic and reliable Advanced Clinical Practitioners to join our Integrated Community Services Division.

The role is part of the Admission Avoidance team and the team works 365 days and the Advanced Practitioners operate 8am to 8pm.

The post is a fixed term contract or secondment until 31/03/24.


The remit of the Integrated Community Service Division is to support the care of patients in the community and reduce the need for them to be admitted to hospital.

Community services are expanding and this is an interesting and exciting time for Advanced Clinical Practitioners to join our team.


The extremely motivated post holders will require well-developed interpersonal and communication skills, They must be able to demonstrate a flexible approach to working and possess the ability to work as an effective member of a multidisciplinary team; whose ultimate aim is to deliver the highest standards of care for all patients and their families.

We offer a mixture of proactive and reactive care for our patients.

The reactive element includes referrals from NWAS paramedics and other community services; whilst the proactive elements offer the opportunity to work with patients for longer periods, promoting independence and self-management of long term conditions and on-going projects around the further development of advanced practice and frailty development in the division.

The Admission Avoidance Team is currently supporting a number of workstreams including Virtual Wards.

The post holder will be expected to contributing to Virtual Ward working by promoting the use of technology alongside advanced clinical assessment and decision making skills to manage patients in their own home.


The post holder will be keen to face challenges, be well organised and enthusiastic in participating with service development through audit and peer review process.

There will be opportunities to contribute to the development of the advanced practice offer throughout the division which includes an intermediate care bed base.


In return for your commitment you will be offered professional support from senior management within the division and regular ACP forums to look at professional issues such as job planning; clinical supervision and reflection; and portfolio building, as well as supporting trainee Advanced Clinical Practitioners.

New Health and Social Care Act Regulations were approved by Parliament on the 22nd July 2021, which requires all those working or entering a CQC registered care home (for persons who require nursing or personal care) to be fully vaccinated (Covid 19) from the 11th November 2021, unless exempt under the regulations.

You will therefore be required to provide proof of vaccination or exemption if you are successful at interview.

Health information is classed as special category data under UK GDPR legislation therefore, vaccination/exemption confirmation will be processed and stored in line with Data Protection Act 2018.
