Description

The Heart Failure Service lead on best practice patient centred care in the management of Heart Failure.

The team working closely together are able to provide collaborative care for patients within the clinical pathway and support health care professionals within the Bristol, North Somerset and South Gloucestershire (BNSSG) region.

This role will work within an administrative team to provide a comprehensive and flexible administrative service to the team of Heart Failure Specialist Nurses, Consultant Cardiologists and Assistant Practitioners working within the Community Heart Failure Service.

The post holder will work collaboratively and flexibly with the team to provide a seamless administrative service ensuring all key functions of the services are covered each day.

The post holder will work well within the team supporting the clinicians, patients and other health care professionals who contact the service.

The service covers the Bristol, North Somerset and South Gloucestershire (BNSSG) geography of Sirona and has a variety of clinics and bases.

The services have different bases within BNSSG, with the main base being at New Friends Hall, Stapleton, Bristol.
