Description
We are looking for an experienced Care Coordinator to join our client's team in the office, located in Peterborough. We are looking for a passionate individual who is eager to grow alongside the expanding company.

The Care Coordinator role plays a vital role within the Home Care team.

As a Care Coordinator, you will have the responsibility of leading a highly efficient team of Care Workers and overseeing the coordination and management of various domiciliary care packages.


Responsibilities of this role include:

  • Coordinating the daytoday running of domiciliary care services
  • Preparing staff rosters
  • Primary point of contact for any enquiries or assistance needed by care staff, customers and their families
  • Oncall duties
  • Process changes to customer care and support plans
  • Managing highly confidential and sensitive information
  • Ensuring clients and Care & Support Workers documentation and records are kept up to date
  • Care plan reviews

Qualification/ Experience

  • Health and Social Care Level
  • Experience in domiciliary homecare
  • Previous Care Coordinator experience
  • You will need to be a driver with your own transport
This is a permanent role with full time or part time hours - minimum 25 hours per week.


Job Types:

Part-time hours: 25 per week


Salary:
£11.75-£12.50 per hour

  • Peterborough, Cambridgeshire: reliably commute or plan to relocate before starting work (required)
