An exciting new opportunity has arisen for a highly motivated and enthusiastic PCN Care Co-ordinator, to work across Derby City as part of our Integrated Community Team.

There is required travel between local GP practices and care homes within the Derby City area.

Key aspects of the role will involve working closely with Community Matrons and wider MDT, identifying and supporting care home residents who require additional support and proactive care planning in line with the Ageing Well agenda and Enhanced Care Home Framework.

The role will also include working closely with Primary Care to co-ordinate MDT meetings with Health and Social Care colleagues.

The successful applicant will have excellent communication skills, be able to work independently and be confident using IT systems.
