A support provider based in
Dorset is looking for a highly motivated and empathic Community Based Project Practitioner to provide assistance for vulnerable women as risk of repeated pregnancy in the
community.
The post holder will provide a necessary and valuable service to the community and help vulnerable women transform their lives in a positive and productive direction; helping them to break the cycle to trauma and improve their wellbeing.

Day to day responsibilities include:
- Provide long term (18 months) personcentred support to a caseload 8 of Female service users who are at risk of repeated pregnancies in the community
- Conduct assessments and primary engagement support for prospective service users, for an initial 16 week period
- Conduct initial assessments and liaise with local authorities prior to initial contact with prospective clients
- Undertake an effective, flexible, intensive outreach approach to engage with users';
- Support the development and maintenance of effective local pathways with key agencies, e.g. sexual health, housing and mental health;
- Ensure an evidence base is applied to practice through a collaborative assessment of a users'needs and strengths
- Develop focused, bespoke and creative interventions that meet the woman's needs and objectives and ensure that any financial spending on women is aligned with their goals as set out in their care plan;
- Ensure outcomes are achieved through the use of the Pause Monitoring, Evaluation and Learning Framework and that data and recording is completed to a high standard;
- Ensure all practice matters relating to child protection or vulnerable adults are dealt with promptly and in line with local safeguarding;
- Represent Pause at relevant events;
- Embed a continuous learning ethos within individual and team practice including involvement in reflective practice discussions and demonstrating and maintaining healthy and professional boundaries
Essential criteria include:
- Current enhanced DBS (within the past 12 months
- Access to a vehicle (travel expenses covered)
