Main Duties & Responsibilities

Working Pattern:

The service operates Monday to Sunday between 08:00 and 20:00 hours and you will be required to work shifts between these times.

Key Areas of Responsibility


To work within a multi-professional team in a community setting, providing, support, care, and treatment to individuals, within guidelines and protocols identified for the role.

To provide support with registering unregistered patients to their nearest General Practice Surgery

To promote and undertake all aspects of the NHS Health Check and ensuring the service is a Making Every Contact Count

To promote the value of immunisations for both adults and children, as well as providing seasonal immunisations for adults including but not limited to COVID 19, Influenza

To assess and sign post patients, or refer complex cases to Social Prescribers, to achieve quality of life and independence where possible

To respond to infectious outbreaks as and when required, inclusive of swabbing and immunising

To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users


To deliver high quality care in the community working with patients towards self-management and independence throughout the continuum of care.

To participate in a team of colleagues supporting evidence based care, facilitating staff learning and development.

To oversee and manage equipment stock on the vans.


To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.

To ensure the needs and wishes of the individual are documented.


Job Types:
Part-time, Temporary contract

Part-time hours: 32.5 per week


Salary:
£11.67 per hour
