Discharge Co-Ordinator - Band - Mental Health Liaison Team:
An exciting opportunity has arisen for a Discharge Co-Ordinator to work within the Mental Health Liaison team.

This vacancy would suit someone who is looking for an exciting opportunity and wants to be part of the wider Multi-Disciplinary Team based at the Royal Stoke University Hospital.


The Discharge Coordinator (DC) will function as part of the Multi-Disciplinary Team (MDT), to help provide a seamless service to patients, with proximal supervision from registered staff and other professionals as appropriate.

They will be skilled in taking the appropriate actions to achieve the planned outcome guided by standard operating procedures and protocols.


One of the most important functions of the DC will be to assist in the timely and safe discharge of patients from the Royal Stoke University Hospital, following an appropriate assessment by the MDT, in line with the relevant policies, care plans and risk assessments.


The DC will be responsible for attending the daily Huddles in the Emergency Department and having a good awareness of the system wide pressures which will then be fed back to the team and senior management.

To liaise daily with the Mental Health Practitioners and Medics to identify patients that require an assessment of their mental health and to ensure patients are discharged in a timely manner.

Responsible for keeping the duty log up to date and inputting any new referrals onto the system.

Using Lorenzo/IPortal to make clinical entries.

Participate in MDT's and handovers.

Plan straightforward tasks guided by standard operating procedures and protocols.

Demonstrate initiative and decision making in relation to prioritising the order patients are assessed.

Support the team in booking patient transport and liaising with other Trusts to organise a safe and seamless transfer.

Basic administration tasks.
