Description

Provide a high quality, responsive person-centred support service to a range of people with complex needs

Build ipen, collaborate and trusting relationships with people accessing the service, including colleagues and partner agencies

Client participation, consultation and involvement to prevent social isolation

Work with individuals to help them to identify specific support needs and draw up a personal support plan; recognising their strengths and development areas and setting timeley objectives where appropriate.

Make informed decisions to identify appropriatesupport.

Complete robust risk assessments and amanage risk considering the specific servicer, individuals, health and safety and policies and procedures

Establish and maintain professional boundaries and safeguarding people.

Collate and assemble information to ensure service targets are met.
