Description
Deliver programmes of clinical care and therapy for treatment as part of the plan of care determined by others.

Deliver clinical/technical interventions and/or therapeutic/rehabilitative/social care to patients based on the individual needs identified, using knowledge and skills of Nurses, Allied Health Professionals and health/social care professionals where required.

To undertake initial assessments of patients following triage-using evidence based care pathways.

Able to plan care using established care pathways and trust clinical protocols Support/undertake the ongoing assessment and management of patient care needs through monitoring of progress, feedback discussions, reports and multidisciplinary meetings.


To work with and alongside relatives and carers (paid and unpaid), sharing knowledge and skills to promote understanding of the aims of intermediate care and/or rehabilitation to ensure a consistent approach to care provision.

Discuss with patients issues relevant to their health and social care needs, based on health promotion and social care good practice.

Manage the daily care needs of a group/caseload of patients, under the direction of the relevant professional.


Undertake review of patient health and social care needs as required against the plan of care, taking appropriate action to achieve the planned aims as directed by the relevant professional.

Requisitions equipment from a limited stock list, following risk assessment, for use by patients, giving patients and carers information for safe usage of such equipment e.g.

pressure relief cushion To assist planned discharge preparation and undertake environmental risk assessment of patient needs, under direction of the relevant professional.

To lead and direct the work of other support workers as required to ensure daily care activities are organised and delivered according to caseload and workload needs and identified priorities.


To contribute towards the facilitation of learning and support of students and trainers as part of their clinical practice placement.

To administer medication as directed by registered professional and within trust guidelines. Communications Communicates daily with other team members, other agencies and professionals e.g. social services and specialist nurses for case discussion or onward referral of patients.


Maintains accurate records of all nursing interventions and other activities associated with work in line with Trust policies and Professional guidelines.

Maintain confidentiality of information. Have the ability to communicate and work with others within the clinical team, maintaining effective working relationships.

Possess the ability and knowledge to utilise different methods of communication with individuals and their families to educate, facilitate and promote desired change.

Able to identify and develop methods to manage barriers to effective communication.

Demonstrate a range of verbal and non-verbal communication tools to communicate complex information effectively with patients, families, colleagues/team members and other agencies.

Possess the ability to discuss with patients and family, issues relevant to their health, therapy and social care. Communicate appropriate information to and from other departments/ service areas as required.


Maintain accurate and comprehensive patient care and treatment records in line with organisation standards of practice and data protection legislation.

Demonstrate empathy with patients, their families and carers, ensuring their views, choices and experiences influence thinking and individualised care provision.

Provide spontaneous and planned advice to non-registered staff, patients, relatives and carers to promote understanding of the aims of the Intermediate Care Service to ensure consistency.

Responsibility for Patient Care Assesses, plans, implements and evaluates an appropriate plan of care for patients, reporting to the District Nurse caseload holder.

Often gives advice and psychological support to seriously ill and dying patients and their carers. Planning and organising Responsible for prioritising own workload. To alert senior nurse on duty of any patient concerns at team time.

Responsibilities for Physical and / or Financial Resources Ensure effective use of material resources/supplies within the team in consultation with senior staff.

Monitor, control and store resources/supplies according to the requirements /specifications of the clinical environment. Participate in ensuring appropriate usage and monitoring of departmental resources.

Responsibility for Policy and Service Development and Implementation Follow strictly the procedures and policy of the trust Actively contribute and participate in the Trusts clinical governance agenda.

To offer constructive views on how the existing service and teamwork can be evaluated and improved upon.
