Job Role:
Medications & Long Term Conditions Administrator


Reports to:
Practice Manager


Job Summary:


To act as a focal point of communication between the GPs, receptionists, practice manager, patients and local pharmacies, as well as other members of the primary health care team, to ensure patients' repeat medication is produced within the current protocols of the surgery.

To arrange appropriate reviews for patients on Repeat medication or those with Chronic Conditions or high risk drug monitoring


Job responsibilities:

To process, all repeat medication requests sent in by patients using the various protocols in place

  • To process repeat medication requests from online sources
  • To collect repeat prescription requests from the fax machine, post box and reception.
  • To liaise with patients and chemists regarding queries and requests.
  • To ensure repeat medication is issued in line with the protocols that are already in place, and ensure that any queries raised are dealt with quickly and correctly.
  • To ensure that patient queries on medication are highlighted to the relevant GP
  • Proactively recall patients for blood tests and reviews, especially those on high risk drugs which need monitoring
  • Follow the prescription policies and processes to continue to improve the service.
  • Arrange blood tests and blood pressure checks prior to medication reviews
  • Arrange home visits for the housebound patients who need routine reviews
  • Deal with patient enquiries relating to medications and prescription issuing
  • Continually assess and evaluate systems recommending changes and improvements.
  • Liaise with medical representatives in accordance with agreed policy
  • Deal with general telephone enquiries from patients and general public.
  • To cover duties at The Dales practice when required.
  • Make appropriate appointments for patients to see Doctor, Nurse and other clinical staff associated with the Practice.
  • Assist in the update of policies and procedures as necessary
  • Assist management to ensure meeting and greeting patients and visitors is of a high standard
  • Assist in generating and promoting a positive work culture, generating ideas to improve services to patients and more effective ways of working
  • Awareness of and working towards QOF and enhanced services targets
  • Processing pathlinks results in accordance with the agreed practice protocol
  • Cytology administration and recalls
  • Arranging referrals to the Diabetes prevention programme
  • Assist in ensuring all administration work allocated is completed efficiently and accurately and that administration work is covered during periods of sickness and holidays
  • Assisting in covering for holidays, sickness and compassionate leave
  • Communicate with the medication management technician for guidance when necessary
  • Cover for reception duties when required

Health & safety:


The post-holder will implement and lead on a full range of promotion and management their own and others' health and safety and infection control as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures.

This will include (but will not be limited to):


  • Using personal security systems within the workplace according to practice guidelines
  • Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines
  • Correct use of Personal Protective Equipment (PPE)
  • Use and monitoring of the correct use of Standard Operating Procedures for cleaning and infection control
