Claremont Hospital are looking for a Ward Clerk to join the Inpatient Ward team, working 25 hours per week on a permanent basis.

JOB DESCRIPTION


You will be required to contribute to the effective performance of the Inpatient areas by consistently providing a high quality, comprehensive administrative and secretarial support service which facilitates the department to meet both local and national requirements and standards.


KEY ACCOUNTABILITIES

  • Provide a comprehensive confidential administrative service to the Department which will include some basic secretarial and personnel work
  • Maintain the integrity of patients case notes and on a daily basis:
  • Ensure sufficient patient demographic labels are available based on the patients expected length of stay
  • File all signed reports (DO NOT file unsigned reports)
  • On discharge ensure all documentation is fled securely and in the correct section of the case note file
  • Return files of discharged patients to the Records Library
  • Return films of discharged patients to Imaging
  • Responsible for the coordination and organisation of patients listed for surgery, sending appointment letters and other relevant documentation in line with appointment schedules determined by the Lead Nurse.
  • Responsible for the collection and preparation of case notes for preadmission assessment attendances ensuring all the relevant paperwork and correct Care Pathway documentation is included ready for the nursing staff.
  • Arrange all outpatient follow up appointments as requested by the nursing staff ensuring the patient is handed their appointment card prior to discharge where possible.
  • Promptly fax letters and other documentation to Primary Care Service as requested by nursing staff
  • Accurately execute the administration of Discharge Surveillance process by ensuring the following:
  • Providing and explaining the documentation to each patient/relative prior to discharge.
  • Maintaining an electronic database which evidences the process and provides an audit trail of responses and actions
  • Provide monthly reports to the Hospital Matron by the 5th working day of the month.
  • Ensure each patient receives a satisfaction survey questionnaire prior to discharge and provide an explanation as to the purpose of the process.
  • Maintain a sound understanding and knowledge of the Hospitals complaints procedure and positively assist patients to raise any concerns they may have and ensure follow up actions are undertaken in a timely manner.
  • As requested, assist the Lead Nurse to undertake audit work as and when required. This may include, but is not restricted to, pulling case notes; completing checklists.
  • Ensure monthly statistics are forwarded to the Hospital Matron by the 5th working day of the month for nutritional screening.
  • Assist the Lead Nurse to maintain any electronic personnel systems as required and undertake any secretarial duties as required by the Lead Nurse.
  • Work flexibly and cooperatively at all times to cover the needs of the service and demonstrate tenacity to follow things through to a successful conclusion.
  • At all times employ safe working practices which fulfil Health and Safety and Infection Prevention requirements and utilise the Hospital reporting system to report all incidents and near misses.
  • At all times maintain own working environment in a clean, neat and decluttered manner and adhere to Infection Prevention procedures and practices so as to proactively contribute to the protection of patients, visitors, and staff from acquiring a healthcare associated infection.
