Description

The Cancer Support Worker team at UHBW are looking for an enthusiastic and motivated person to join our compassionate & well-established team, who provide personalised support and information to people living with and affected by cancer.


In line with the NHS Long Term Plan, UHBW are implementing a digital cancer remote monitoring system, My Medical Record (MMR), to enable remote cancer surveillance and Personalised Stratified Follow-Up (PSFU) pathways of care for people with cancer.

MMR allows patients and clinical staff to manage follow ups remotely, via an on-line patient portal and clinical tracker, as well as health and wellbeing resources and direct access to their clinical team.

This Cancer Support Worker role will be critical in the effective implementation and development of My Medical Record services for cancer follow-up care.


Our cancer support workers work closely with all members of the cancer clinical teams and cancer services across the Trust, providing coordination & delivery of support and signposting to additional services.

This provides a fantastic opportunity for you to contribute to improving the overall experience of cancer care at UHBW and work within a friendly and supportive team with an interdisciplinary approach to delivering personalised care.


  • Act as a single point of access to patients who are on selfmanaged followup pathways
  • Proactively support and coordinate the overall needs of patients on triggered followup pathway, using own judgement and escalating to the relevant practitioner as necessary
  • Support and coordination of remote surveillance for patients on self managed follow up (PTFU) pathways to ensure risk free survivorship
  • Coordinate access to personalised information and education resources for patients, and their carers, on PSFU pathways
  • Coordinate and support access to and delivery of self management programmes for patients and their carers
  • Communicate effectively with patients and carers and with a wide range of clinical and nonclinical stakeholders in secondary and community settings
  • Organisation and Planning
