Description
East Kennet Primary Care Network are seeking a Non-Clinical Care Coordinator. The role will primarily be based at Kennet & Avon Medical Partnership, Marlborough and Ramsbury & Wanborough Surgery, Ramsbury.


You will be required to provide support to patients, their families and carers, signposting, supporting and navigating through available local services.
