Description
The Community Dietetic team is responsible for providing dietetic advice in a diverse range of nutritional areas.

To support to a caseload of adults and their carers in a range of settings including clinics, nursing homes, patients own homes, as well as remotely.

We have specialist Dietitians in Gastroenterology, Home Enteral Feeding, and Paediatrics. There are also Dietitians working with Learning Disabilities, Neuro Rehab and Diabetes with whom we share training and education.


There are excellent IT facilities in all offices and you will be assigned a laptop and phone to facilitate remote working.


We have an excellent administration team who undertake all clerical work leaving the dietitians free to maintain and develop clinical services.


KEY WORKING RELATIONSHIPS:

To work within the community dietetic therapy team in Hounslow.

Link with other healthcare teams e.g.

Hounslow Community Recovery service

Integrated Community Response service.

Speech and Language Therapy Service

GPs

Colleagues in referring acute hospitals

Community Nursing teams

Nursing homes

Cardiac rehab services

Practice Nurses

Medicines Management


JOB PURPOSE:


The post holder will:


  • Provide a service to patients in the Community and in Residential and Nursing Homes who are on Oral Nutrition Support and HEF patients (when required).
To train and education GP's and other Practice and Community staff on Oral Nutrition Support policies and procedures

The post holder will cover as part of the Cardiac Rehabilitation team to provide an effective Dietetic cardiac rehab service to staff, carers and those patients referred

To contribute to the Community Out Patient service across Hounslow and Richmond

To support the In Patient service to the wards at Teddington Memorial (Community) Hospital
