£10.05 (probation) rising to £10.25


Overview:


Responsibilities:


  • To work with the individual to help them develop practical and personal skills to help them realize their potential.
  • To provide a supportive service which meets the individual's needs.
  • To support individuals to pursue hobbies, attend clubs and have an active social life.
  • To support individuals to attend educational services, work, day services or a college/further education placement.
  • To support individuals to utilize community facilities.
  • Assist in the development of each individual and sensitively support their personal care needs and relationships
  • When required to administer prescribed medication as per our Clients' medication policy and procedure.
  • To ensure that all agreed care plans, assessments, management strategies, and behavioral interventions are implemented as agreed.
  • To ensure that all recording forms and care plans are completed.
  • Support individuals during specific treatment, therapeutic and development program's as prescribed, by appropriate clinicians.
  • To liaise with others involved with care planning, e.g., parents/carers, Social Worker, schools, GP
  • In support of the Home Manager, encourage and motivate colleagues in meeting the needs of the individual and to lead by example and be a great role model.
  • To be aware of and act within the County's and Clients' Safeguarding and Vulnerable Adults Policy and Procedure.
  • To be an equitable part of the rota to provide a 'stand by' service to those individuals for whom you are a nominated part of their team

Team Participation:


  • Assist the Home Manager in recruitment and selection by taking an active role in supporting new staff.
  • Contribute to the development of other team members.
  • Support the effective functioning of the team by ensuring that good relationships with colleagues are established and maintained and by adopting a constructive, reflective approach.
  • Support the creation of your Personal Development Plan and be committed to learning.
  • Create and follow activity plans for individuals as required.
  • Identify, minimize and report to the line manager any interpersonal conflict which arises within the team.
  • Ensure that colleagues always adhere to Safeguarding policy and Speak Up policy and report to Home Manager or upwards any concerns in this area in relation to colleagues.
