  • Maintain a regular onsite presence within the surgery, attend practice/ clinical meetings and promote a supportive atmosphere in the workplace
  • Consistently manage an appropriate social prescribing caseload as agreed with the practice to meet the requirements of the PCN DES
  • Contribute to the extended access requirements of the PCN in line with other professional groups
  • Maintain accurate clinical records of all patient consultations and related work.
  • Promote health coaching and social prescribing amongst the PCN and networked practices and to support colleagues to improve their skills and understanding of personalised care.
  • Support patients with low or medium level mental health needs, in 1:1 or group sessions, in person or remotely.
  • Provide brief personalised support to individuals, their families, and carers to ensure that they are active participants in their own healthcare and to empower them to take more control in managing their own health and wellbeing, to live independently and to improve their health outcomes through the following:
  • Providing interventions such as self-management, education, and peer support
  • To actively signpost patients to the correct healthcare professional.
  • Supporting people to establish and attain goals set by the person based on what is important to them, building on goals that are important to the individual
  • Coach and motivate patients through multiple sessions to identify their needs set goals and support them to develop knowledge, skills and confidence to become active participants in their care.
  • Undertake holistic assessments and codesign Health and Wellbeing Plans with individuals, identifying support needs to ensure maximum engagement in improving health and wellbeing.
  • Provide patients with continuity and a coordinated experience of care, remaining the point of contact throughout the individual's social prescription/ health coaching sessions.
  • Train and develop GPs and primary care health teams' knowledge on how to identify patients suitable for social prescribing service referral
  • Establish and maintain strong working relationships with social prescribing link workers and health coaches across the organisation, offering advice, support, training and mentorship.
  • Develop and facilitate patient selfmanagement and wellbeing workshops and peertopeer support groups
  • Develop and deliver organisational training modules that will as a minimum cover the role of social prescriber and health & wellbeing coaches, eligibility criteria and how to differentiate between low & high needs, patient experience and common themes/feedback from patients
  • Deliver a minimum of 4 patient engagement community events annually which will encourage a wider range of patients to access social prescribing services thus reducing inequalities
  • Work innovatively with the practice leadership teams to engage difficult to reach patients with a view to increasing their activation levels and improving engagement
  • Build links with wider community 3rd sector organisations and build a directory of community assets for easy access by patients and staff alike
  • To ensure information on sources of voluntary and community support is up to date at all times to enable effective and accurate signposting and linking of individuals with services.
  • Provide quarterly comprehensive outcome focused reports detailing the progress of the service
  • To be considered for this exciting role you must have:
  • A caring, kind, empathetic with excellent listening and communication skills
  • A health, social care, counselling or other relevant professional or academic qualification.
  • Excellent holistic assessment skills and experience of shared decisionmaking providing support to adults in a planned and structured way to improve health, recovery and wellbeing outcomes.
  • Experience of training, supervising, motivating and developing others to ensure a highquality service that meets the needs and promotes positive outcomes for individual service users.
  • Proven skills in collating information and data on community resources and organising these in uptodate and accessible formats for a range of different service users from various communities.
