An exciting opportunity has arisen for an enthusiastic and motivated data and information officer to work across the two teams providing Community Stroke Rehabilitation and Therapy Support for Coventry and Warwickshire.

These Integrated Community Stroke Services provides therapy support to people who have recently left hospital, following a diagnosis of stroke.


To collect, retrieve and enter patient data from a variety of sources in order to facilitate Clinical Audit and data collect within Stroke Services.


They will be required to have an advanced knowledge of excel and other computer systems, with an interest in data and how data can be used to develop a service.


The role will include working with patient notes, so an understanding of medical terminology would be beneficial, with the ability and willingness to learn the specialist's terminology.

To collect and input data for patients who are to be included in the post-acute organisational audit (PAOA) SSNAP (Sentinel Stroke National Audit Programme).


To collect and input patient data relating to community activity onto a variety of databases within the Stroke Service in Warwickshire and Coventry including the services own Stroke database.

To collect and input data relating to the stroke pathway.


To complete upload of patient data from Capture Stroke to SSNAP on a weekly basis ensuring any data errors flagged up as part of this process are resolved.

To identify missing data and use a variety of sources, including EMIS to complete records.


Work closely with clinical and admin staff within the Warwickshire and Coventry stroke service to reduce the occurrence of missing data and improve data quality.

To ensure data is uploaded in a complete, timely and accurate manner.

Liaise professionally with all departments regarding admissions and discharges to ensure all data is correct.

To escalate coding discrepancies within the clinical teams.

To report on and present data to a variety of staff (e.g., TIA, Stroke discharges).

To attend the SSNAP Performance meetings.

To develop and maintain effective working relationships with colleagues both internally and externally as required.

To work closely with the Community Stroke Team Manager to ensure outstanding work and priorities are met.

To ensure compliance with all Trust policies and procedures.

Embed changes and improvements to data collection and reporting systems in collaboration with clinical teams.

To attend all required mandatory training sessions.
