We are currently looking to recruit 2 x enthusiastic and motivated Health and Lifestyle Practitioners to join the 'Be Well' Health and Wellbeing Team within primary care.


One practitioner will work across theVenn PCN in Hullcovering Hull and one practitioner will work across theBeverley PCN in Beverleycovering the area of Beverley and Cottingham.

The 'Be Well' team consists of Heath & Lifestyle Practitioners, Community Link Workers and a Trainee Health Psychologist.

This will be a central role within the team providing support to both practice staff and patients within all GP practices across the entire Venn/Beverly PCNs.


This is an innovative role designed to work in an outcome focused way to improve the quality of life, and health and wellbeing of local patients, by recognising that this can be affected by a wide range of social, economic and environmental factors.

The post holder will support patients to achieve their personal aspirations and participate in their local and wider communities, enhancing effective personal support networks, and enabling individuals to maintain healthy lifestyles.

The post holder will be expected to identify regular signposting and referral opportunities for additional support.

This may include to other roles within the 'Be Well' team but may also include referring/signposting to other services and outside agencies, utilising key elements of the Making Every Contact Count (MECC) national agenda.


The post holder will be responsible for ensuring accurate and up to date data is added to our computerised systems (SystmOne, Excel and other databases as required).

You must have an excellent knowledge of Microsoft packages and be confident to use these on a daily basis, therefore good keyboard skills are essential.

In addition to IT experience, there is a need for excellent customer service and good communication skills as well as a confident telephone manner.

The post holder will also play in an important role in supporting an ongoing service evaluation.


You will need to have an understanding of confidentiality and data protection, and a high level of accuracy and attention to detail is paramount.

You must have a flexible approach to work in order to deal with set deadlines and varying workloads, and the ability to prioritise your own caseload.

You will be working at the GP practices within your respective PCN, so the ability to travel around within your dedicated area is essential.
