Rate - £24 per hour - inside ir35

Your Role

  • We are seeking an experienced Housing and Benefit expert to work in an exciting new team to help support hospital discharge. We are establishing a move on team which will work with patient who have moved out of hospital into one of a number of Council funded interim placements because they have not been able to return back to their own homes for a number of reasons. This cohort is both working age and older adults who have homes but which are unsuitable or unsafe. It will not cover homeless client which are supported by another pathway. You will work with a team of OTs and Social Workers to support these clients to be able to go back to their own homes or move to more appropriate accommodation including housing with care. This is a pilot but we have commissioned an independent review to assess the discharge pathway, including this new move on team which may result in longer term appointments.
