You would be joining a team of four band 3 administrative staff and one other band 4 Referral and Administration coordinator.

The Administration team provide administrative support across 3 clinical teams in addition to registering referrals to the service.

Our bases are independent SHSC buildings in Upperthorpe (staff base and outpatient facility) and on the Michael Carlisle site (staff base only).

The teams consist of nurses/case managers; occupational therapists, physiotherapists, psychologists, speech and language therapists and therapy assistants. LTNC services are community based.


We want you to be part of ongoing reflection and service development / quality improvement and optimising the patient experience and outcomes.

There is a strong commitment to PPI work and co-production; In service training programme and supervision.


You will need to have strong administrative skills; experience of managing referrals and electronic patient records (preferably TPP/Systm One); experience of managing a team of admin staff - including supervision, PDRs etc some knowledge/experience of working with people with long term conditions and relevant other services within Sheffield would be advantageous.

To act as the central point of contact for all referrals into the LTNC Referral Point.


To manage and triage referrals to LTNC services, including onward referral to the receiving LTNC teams and other clinical provider service, with clinical support.

To support the LTNC Operational Manager with administrative tasks.

Act as a building manager for LTNC sites

To organise workload within the LTNC services and co-ordinate activities to ensure that an efficient service is provided.

The post holder will be expected to monitor and assess workload amongst the team, including being able to identify and step-in to fill gaps, to ensure continuity of services.

It is essential that the post holder is able to exercise initiative suitable to the role and is fully supportive to other colleagues in the administrative team.


Be responsible for daily workload management, supervision and appraisal of junior staff to support the overall function of the admin team.

General requirements


Work on identified strategic admin areas to ensure that these are embedded within the working practices of the admin team which will ensure an efficient administrative support for the LTNC services.

Have expertise in developing and managing office systems across the LTNC services.


Tasks will include:

Using the full Microsoft Office suite to full advantage, and particularly Word, Power Point and Excel packages.

Use of TPP/SystmOne patient information system.


Use the best techniques in IT and best practice to ensure high quality management information and administration systems are in place.

Principal duties


Referral Coordinator duties:


Act as the central contact and resource for internal and external professionals, service users and relatives/carers accessing appropriate LTNC Services.

Manage referrals

To ensure all referrals are registered on SystmOne Referral Point Unit and transferred to the appropriate LTNC service.


Support the Operational manager in ensuring that the admin team proactively seek information pursuant to the completion of referral data or initial risk assessment.


This may include direct verbal and written communication with statutory and non-statutory health and social care providers, service users and carers, police, members of the legal professions, amongst others.

Inform referrer of outcome of referral.


To be alert to any safeguarding/risk issues indicated at time of referral and ensure that this information is passed on appropriately to the receiving LTNC service.


To oversee, receive and act upon enquiries about referrals from service users, carers, statutory and non statutory providers via face to face contact, over the telephone, or via written communication.

To follow a proforma to conduct triage of referrals for SCBIRT - with carer and significant others - in order to be able
